Simply click the Online Reporting button and indicate whether it is an automobile loss, liability loss, property loss or worker's compensation loss, select the form you need, fill it out, and click the "submit" button. A Lighthouse Group Claims Representative will respond to you and confirm that your claim has been received. Please forward any supporting documentation via fax or e-mail using the information provided below.

If your confirmation has not been received within the next business day, please contact us immediatly to verify that your loss was forwarded to us correctly. Not certain what type of loss you have?
